Social Determinants of Health: Methods of Success - WHITEPAPER

Download the exclusive whitepaper

Medicaid Services and other managed care organizations (MCOs) have recently shifted away from providing their beneficiaries with plans that suit only care for acute and chronic medical conditions in clinical settings. Many have made provisions to include particular non-medical services into their plans as a means not only to ameliorate the risk of chronic disease in their members, but to reduce the funding these members require as their conditions continue to deteriorate, which leads to increase hospital admissions, specialist visits and other costly medical services.

The provision of these non-medical services, which aim to create awareness around and address social determinants of health, as well as their benefits, has been of great debate among policymakers and MCOs alike. This whitepaper presents an overview of social determinants of health, including:

  • Successful SDoH implementation case study: HPP and MANNA

  • Case studies of how Medicaid Programs are incentivizing MCOs to include SDoH work

  • Creating social and physical environments that promote better health

  • Community based organizations managing SDoH

  • Federal/State updates allowing MCOs to include SDoH benefits

  • Funding of SDoH
