Insurance Requirements
A Certificate of Insurance (COI) is required for ALL sponsors who are showcasing and/or displaying at Center Store Forum.
A COI must be on file by Friday, August 29, 2025.
Certificate of Insurance Requirements:
- Name of Insured: Must be the legal name of contracting party/Company Name (the one who is purchasing the insurance)
- Commercial General Liability: $1,000,000 per occurrence
- Workers Compensation & Employers' Liability: Must meet statutory requirements.
- Hotel to be listed as additional insured: Lombard Public Facilities Corporation d/b/a The Westin Chicago Lombard 70 Yorktown Shopping Center, Lombard, IL 60148, USA
- COI must be valid for the duration of the event
